By signing this form, I authorize MYPCSVIP to process my credit for the service selected above. I understand that there will be a monthly recurring charge for the chosen Membership or Package. This agreement operates on a month-to-month basis, and I retain the right to cancel at any time. MYPCSVIP will not be liable for any services if I decide to cancel my Membership Package.

24-hour Refund Policy: If I decide to cancel my membership within 24 hours, I am entitled to a full refund credited back to my credit card. I acknowledge that any cancellation made after 24 hours from signing this agreement will not be refunded, as work on my website project will have already begun.

Verbal Approval: In the event an Associate Agent helps me complete the agreement via phone, I grant permission for the Agent to type my name and use my initials for an electronic signature. I will receive a copy of the finalized agreement on the date it was completed. The date will serve as confirmation of my authorization for the electronic signature once it is emailed to me. Upon receiving it, I will reply with:

Modifications to this agreement will not be valid unless they are in written form and signed by both parties. This agreement is binding and advantageous to both parties and their successors. Timeliness is crucial to this agreement. This document, along with any attachments, constitutes the whole agreement between the parties. The laws governing this agreement are those of the state in which it was signed.
